Before you schedule, please note the following...
- When you complete the online form, you will be immediately added to our schedule, and we will be looking forward to seeing you! Prior to your visit, we will need more information (i.e. insurance policy number, etc.) from you. You should receive a call from us prior to your appointment. Please bring your insurance information, medication list, and any glasses or contacts you wear with you to your appointment.
- Please arrive 15 minutes prior to your scheduled appointment so there is time to add your additional data to your chart as well as give you time to review our HIPAA and Office Policy forms. This helps ensure your appointment starts on time.
- We perform Optomap Retinal Screenings on every patient annually. These photos typically prevent the patient from needing to be dilated during their exam, and allows us to find and document pathology. Insurance does not cover this screening photo and will be your responsibility ($35) at the time of the exam.
- Due to significantly higher patient volume on Saturdays, if you have a history of pathology or would like to have more time discussing your issues or concerns with the doctor, we recommend scheduling on a weekday.
